Quick answer

A bulbous tip is a nose tip that looks round, wide or boxy instead of defined. It comes from the shape of the tip cartilages, the angle between them, and the thickness of the skin. It is refined with tip rhinoplasty: the cartilages are reshaped mainly with stitches, trimmed conservatively, and supported so the tip stays strong. Tip swelling settles last — the final result takes 12–18 months, longer with thick skin.

What is a bulbous tip?

The tip is shaped by two small curved cartilages, one on each side (the lower lateral cartilages). When they are wide, strongly curved or set far apart, the tip looks round. Thick, oily skin softens the outline further. Surgeons often distinguish a round 'bulbous' tip from a square 'boxy' one, because the fix differs slightly.

BulbousA round tip from wide, convex cartilages.
BoxyA square tip from a wide angle between the two domes.
Thick-skinnedNormal cartilage under thick skin — definition is limited by the skin.
Weak, under-supportedA soft tip that spreads and droops.
Fleshy alaeFull, rounded nostril wings from a deep fat pocket in the alar crease.

Understanding the anatomy

Dr Sanjay

A bulbous or round tip is one of the most common problems I see in my practice. It usually comes from one or more of three things: thick, oily (sebaceous) skin, common in Asian and acne-prone skin, which often needs treatment before surgery with isotretinoin tablets or cream; a thick fat layer (SMAS) over the cartilage, which I remove judiciously to thin the tip and the nostril wings; and broad, flat tip cartilages, which need strengthening with stitches or grafts.

Labelled diagram of the nose from the side and front: nasal bone, upper lateral cartilage, septal cartilage, lower lateral (alar) cartilage, nostril (ala) and the soft tissue envelope
The structure of the nose. The nasal bone forms the upper third. Below it, the upper lateral cartilages and the lower lateral (alar) cartilages shape the middle and the tip, supported in the midline by the septal cartilage. Everything is covered by the soft tissue envelope, and the nostril (ala) is mostly soft tissue.
What you seeWhat's underneathWhat changes
A round, full tipWide, convex tip cartilagesDome-shaping sutures; conservative trimming of the upper edge
A square, boxy tipA wide angle between the two domesSutures that bring the domes closer
A tip that looks heavy but softThick skin and a thick fibro-fatty (SMAS) layer over the cartilageSMAS trimming — careful thinning of this layer; taping and steroid injections after surgery
Full, fleshy nostril wingsA deep fat pocket in the alar crease and bulky ("chubby") alaeRemoval of the deep fat pocket and reduction of the alae
A tip that spreads when you smileWeak tip supportA columellar strut or septal extension graft

Why not simply remove more cartilage? Over-trimming weakens the tip. Over time it can pinch, collapse or pull the nostril rims upward. Modern tip surgery relies on stitches and support, keeping a strong strip of cartilage at the rim.

Is my tip bulbous? A quick self-check

  • From the front, the tip looks round rather than having two soft highlights
  • The tip looks wider than the bridge above it
  • From below, the tip looks square or trapezoid
  • Your skin over the tip is thick or oily

What are my treatment options?

OptionWhat it doesWhat it can't doRecoveryHow long it lasts
Tip rhinoplastyReshapes and supports the tip cartilages; often through a closed approachCannot make very thick skin look thin5–10 days social; 12–18 months finalLong-lasting — but the tip keeps changing with age (see below)
Full rhinoplastyRefines the tip together with the bridge or widthLonger operation and recovery7–10 days social; 12–18 months finalLong-lasting — the tip still ages
FillerNot suitable — filler adds volume to a round tipCannot narrow the tip; the tip is a higher-risk area for filler——

Your nose keeps ageing. The result of tip surgery is long-lasting, but the nose continues to change with age: the skin becomes thicker or thinner, fat increases, cartilage weakens and the tip droops. Good support at surgery slows these changes; it cannot stop them.

How is tip refinement done?

  1. Planning: front and base views map the width, shape and skin.
  2. Approach: Dr Sanjay often prefers an open tip approach, which lets him remove the fat layers carefully and shape the cartilages precisely.
  3. Conservative trimming: a narrow strip of the upper edge of each cartilage may be removed, leaving a strong rim strip.
  4. Suturing: support stitches called domal sutures narrow and define each side; interdomal sutures bring the two sides together.
  5. Support: when needed, a tip support graft (shield graft), a columellar graft or a septal extension graft keeps the tip from drooping or spreading.
  6. Soft tissue (SMAS) trimming: when the skin is thick, the fibro-fatty layer over the tip is thinned carefully so the new shape shows through.
  7. Alae: when the nostril wings are full, the deep fat pocket in the alar crease is removed and chubby alae are reduced.
  8. Finish: dissolvable stitches and taping; a splint if needed.

Recovery: what to expect week by week

After tip surgery alone most people are back at work in 5–7 days. The tip is the last area to settle: it stays firm and slightly full for months, and the final definition takes 12–18 months. Timings are typical, not guaranteed.

WhenWhat happensWhat you can do
Day 1Nose feels blocked; mild pain controlled with tablets; splint onRest with head raised; usually home the next day
Days 2–5Swelling and bruising around the eyes peak, then fadeShort walks; no bending or lifting
Day 7Splint and any stitches removedSee your nose for the first time; still swollen
Days 10–14Most bruising goneBack to work and social life
Weeks 3–4Swelling keeps fallingLight exercise
Week 6Bones are stableGym, swimming, glasses resting on the nose
Month 3About 80% of swelling goneNose looks natural to others
Month 12Tip swelling fully settledFinal result; review photos

Specific to tip refinement:

  • The tip feels firm and numb for several weeks — this is expected
  • Night-time taping and, if needed, small steroid injections help tip swelling settle
  • Final definition often takes 12–18 months, longer with thick skin

Dubai-specific tips: avoid direct sun and the beach for 4–5 weeks, use SPF on the nose, and plan around Ramadan fasting and the summer heat. International patients: plan a minimum stay of 7–8 days in Dubai before flying home.

Risks, and how they are reduced

Every operation carries risk. These are the ones that matter most for this problem, and how they are reduced.

RiskWhat it meansHow it is reduced
Pinched or over-narrowed tipA tip that looks pinched or collapses over timeSuture-based shaping; leaving a strong rim of cartilage
Nostril rim retraction or notchingThe nostril edge lifts, showing more of the insideConservative trimming; rim grafts when needed
Tip asymmetryOne side of the tip slightly differentSymmetric suturing; checks during surgery
Fullness above the tipA 'pollybeak' from swelling or scar tissueCorrect support; taping; steroid injections
Bleeding or infectionUncommon; usually minorStop blood-thinning medicines and supplements before surgery; sterile, accredited theatre
Prolonged swellingEspecially with thick skin; the tip settles lastTaping, and steroid injections when needed
Numbness, scarring, asymmetryTemporary tip numbness; a fine scar with the open approach; minor asymmetryExplained at consultation
Breathing changesRarely, breathing feels worseAssess the airway before surgery; keep or add support to the nasal valves
Need for revisionA second, usually smaller, operationRealistic planning and staged follow-up
Anaesthesia risksRare in healthy patientsPre-operative tests; certified anaesthetist

Results vary between individuals and cannot be guaranteed. Smoking, some medicines and certain medical conditions increase risk.

How much does tip refinement cost in Dubai?

Tip refinementAt consultationWritten, itemised quote after consultation

The fee for tip refinement depends on what needs to change, whether grafts are needed, and whether it is a first or revision operation. You receive a written, itemised quote after consultation.

What changes the price:

  • Tip alone, or full rhinoplasty
  • Whether grafts are needed
  • Thick-skin management
  • Primary vs revision surgery

For context, rhinoplasty in Dubai generally ranges from about AED 15,000 to AED 65,000. Payment options are available on request.

Frequently asked questions

Why does the tip take so long to settle?
The tip skin is thicker and holds swelling longest. It feels firm and may look full for several months, and final definition often takes 12–18 months.
Can a thick-skinned tip be refined?
Yes, but definition is limited by the skin. A strong, well-supported framework shows through thick skin better than a reduced one.
Can tip surgery be done without touching the bridge?
Yes, if the bridge is already in proportion.
Will my tip droop later?
Tip support grafts are used to reduce that risk. Some settling over the first year is normal.
Will there be visible scars?
With a closed approach there are no external scars. With an open approach there is a small scar across the skin between the nostrils, which usually fades to be barely noticeable.
How painful is it, and when can I go back to work?
Most patients describe discomfort and a blocked feeling rather than pain, controlled with tablets. Most return to desk work in 7–10 days.
When can I fly home after surgery?
The splint is removed on day 7 and the final check is on day 8, after which you can usually fly.
What is the right age for rhinoplasty?
Usually from about 18 in girls and 18–19 in boys, once the nose has finished growing — unless there is a severe airway problem.
How do I prepare for my consultation?
Bring front and side photos, a list of your medicines and supplements, and any previous nose surgery reports. Stop smoking at least 2 weeks before surgery.
