A droopy (hanging) tip points downwards, so the angle between the nose and upper lip is closed. It can be inherited, come from weak tip support or a long septum, develop with age, or appear only when you smile because of a small muscle that pulls the tip down. It is corrected with tip rhinoplasty that lifts (rotates) the tip and supports it with a graft so it stays up. Most people return to desk work in 7–10 days.
What is a droopy tip?
In profile, the tip normally points slightly upwards. The angle between the columella (the skin between the nostrils) and the upper lip is often around 90–95° in men and a little more open in women, though ideals vary between faces and backgrounds. A droopy tip closes this angle and makes the nose look longer and a hump look bigger.
Understanding the anatomy
| What you see | What's underneath | What changes |
|---|---|---|
| The tip points downwards | Weak inner (medial) tip cartilages | A columellar strut or septal extension graft lifts and supports the tip |
| The nose looks long and the tip hangs | A long lower end of the septum | Conservative trimming of the lower septum; rotation of the tip |
| The tip dips when you smile | A strong depressor septi nasi muscle | The muscle is released or repositioned during surgery |
| The droop has increased with age | Stretched ligaments and softer cartilage | Rebuilding support; sometimes tightening the tip ligaments |
Is my tip droopy? A quick self-check
- In profile, your tip points level or downwards rather than slightly up
- Your tip dips noticeably when you smile
- Pressing your tip up slightly with a finger makes your whole face look lighter
- Your nose looks longer than it did a few years ago

| Option | What it does | What it can't do | Recovery | How long it lasts |
|---|---|---|---|---|
| Tip rhinoplasty (rotation and support) | Lifts the tip and supports it with a graft; can release the smile muscle | Cannot make a very long nose short on its own — see the long nose page | 7–10 days social; 12 months final | Permanent, though ageing continues |
| Full rhinoplasty | Combines tip lift with hump reduction or shortening | Longer operation and recovery | 7–10 days social; 12 months final | Permanent |
| Botulinum toxin (non-surgical) | Relaxes the muscle that pulls the tip down when smiling | Only helps smile droop; wears off | Same day | About 3–4 months |
| Filler (non-surgical) | Can prop up the tip slightly at the base of the columella | Limited, temporary lift; rare but serious risk of blocked blood vessels | Same day | Months |
How is a droopy tip lifted?
- Planning: the target tip angle is set for your face, gender and profile.
- Approach: usually open, because the tip needs precise support.
- Support: a septal extension graft or columellar strut from your septal cartilage sets the new tip position.
- Rotation: the tip cartilages are secured to the support in the lifted position.
- Length: if the septum is long, its lower end is trimmed conservatively.
- Smile droop: the depressor septi nasi muscle is released or repositioned if it pulls the tip down.
- Finish: dissolvable stitches, taping and a splint for about a week.

Recovery: what to expect week by week
Most people are back at a desk job in 7–10 days; the final shape takes about a year to settle. Timings are typical, not guaranteed.
| When | What happens | What you can do |
|---|---|---|
| Day 1 | Nose feels blocked; mild pain controlled with tablets; splint on | Rest with head raised; usually home the next day |
| Days 2–5 | Swelling and bruising around the eyes peak, then fade | Short walks; no bending or lifting |
| Day 7 | Splint and any stitches removed | See your nose for the first time; still swollen |
| Days 10–14 | Most bruising gone | Back to work and social life |
| Weeks 3–4 | Swelling keeps falling | Light exercise |
| Week 6 | Bones are stable | Gym, swimming, glasses resting on the nose |
| Month 3 | About 80% of swelling gone | Nose looks natural to others |
| Month 12 | Tip swelling fully settled | Final result; review photos |
Specific to tip lift:
- After muscle release the upper lip can feel stiff and the smile slightly different for a few weeks
- The tip sits a little higher at first and settles slightly over the first months
Dubai-specific tips: avoid direct sun and the beach for 4–5 weeks, use SPF on the nose, and plan around Ramadan fasting and the summer heat. International patients: plan a minimum stay of 7–8 days in Dubai before flying home.
Risks, and how they are reduced
Every operation carries risk. These are the ones that matter most for this problem, and how they are reduced.
| Risk | What it means | How it is reduced |
|---|---|---|
| Over-rotation | A tip that turns up too much, showing the nostrils | Measured rotation; a natural angle for your face |
| Loss of lift over time | Some settling of the tip in the first year | Firm support grafts |
| Stiff tip or upper lip | The tip feels firm; the smile feels different for a few weeks | Usually settles; explained beforehand |
| Bleeding or infection | Uncommon; usually minor | Stop blood-thinning medicines and supplements before surgery; sterile, accredited theatre |
| Prolonged swelling | Especially with thick skin; the tip settles last | Taping, and steroid injections when needed |
| Numbness, scarring, asymmetry | Temporary tip numbness; a fine scar with the open approach; minor asymmetry | Explained at consultation |
| Breathing changes | Rarely, breathing feels worse | Assess the airway before surgery; keep or add support to the nasal valves |
| Need for revision | A second, usually smaller, operation | Realistic planning and staged follow-up |
| Anaesthesia risks | Rare in healthy patients | Pre-operative tests; certified anaesthetist |
Results vary between individuals and cannot be guaranteed. Smoking, some medicines and certain medical conditions increase risk.
How much does droopy tip surgery cost in Dubai?
